Kaiser humira copay assistance11/10/2023 Kroger canceled its pharmacy contract with Express Scripts last fall, saying it had no bargaining power in the arrangement, and, on Aug. The Federal Trade Commission began a major probe of the companies last year. Yet challenges to PBM practices are mounting. The leading PBMs - Express Scripts, Optum, and CVS Caremark - are powerful players, each part of a giant health conglomerate that includes a leading insurer, specialty pharmacies, doctors’ offices, and other businesses, some of them based overseas for tax advantages. Although rebate numbers are kept secret and vary widely, some reportedly jumped this year by 40% to 60% of the drug’s list price. Industry sources also say the PBMs persuaded AbbVie to increase its Humira rebates - the end-of-the-year payments, based on total use of the drug, which are mostly passed along by the PBMs to the health plan sponsors. In other words, dropping one AbbVie drug would lead to higher costs for others. One of AbbVie’s strategies was to warn health plans that if they recommended biosimilars over Humira they would lose rebates on purchases of Skyrizi and Rinvoq, two drugs with no generic imitators that are each listed at about $120,000 a year, according to PBM officials. “We are competing very effectively with the various biosimilar offerings,” AbbVie CEO Richard Gonzalez said during an earnings call. The drugmaker has told investors it doesn’t expect to lose much market share through 2024. The company’s fight for Humira didn’t stop when the biosimilars finally appeared. Its manufacturer, AbbVie, has aggressively defended the drug, filing more than 240 patents and deploying legal threats and tweaks to the product to keep patent protections and competitors at bay. Humira’s $200 billion in global sales make it the best-selling drug in history. The drugs are increasingly important in treating cancers and autoimmune diseases, such as rheumatoid arthritis and inflammatory bowel disease, that afflict about 1 in 10 Americans. health care spending, with their costs increasing 12.5% annually over the past five years. health care system, whose prescription drug coverage can be spotty and expenditures far more unequal than in other advanced economies.īiologics like Humira occupy a growing share of U.S. The ongoing saga of Humira - its peculiar appeal to drug middlemen and insurers, the patients who’ve benefited, the patients who’ve suffered as its list price jumped sixfold since 2003 - exemplifies the convoluted U.S. “We’re not seeing a lot of takeup of the biosimilar,” said Keith Athow, pharmacy director for Tennessee’s group insurance program, which covers 292,000 state and local employees and their dependents. That gives doctors and patients little incentive to switch. Express Scripts and Optum Rx, two of the three giant PBMs, have put biosimilars on their formularies, but at the same price as Humira. The business won’t work unless they gain significant market share, they say. While a manufacturer may need to spend a few million dollars to get a generic pill ready to market, makers of biosimilars say their development can require up to eight years and $200 million. “The percentage savings might be similar, but the total dollar savings are much bigger,” Holmes said, “as long as the plan sponsors, the employers, realize the opportunity.” Coherus BioSciences launched its Humira biosimilar, Yusimry, in July at $995 per two-syringe carton, compared with Humira’s $6,600 list price for a nearly identical product. That’s because biologics are much more expensive than pills and other formulations made through simpler chemical processes.įor example, after the first generics for the blockbuster anti-reflux drug Nexium hit the market in 2015, they cost around $10 a month, compared with Nexium’s $100 price tag. With dozens of biologics nearing the end of their patent protection in the next two decades, biosimilars could generate much higher savings than generics, said Paul Holmes, a partner at Williams Barber Morel who works with self-insured health plans. Biosimilars Are Cheaperīiologics - both the brand-name drugs and their imitators, or biosimilars - are made with living cells, such as yeast or bacteria. states, switched most of its patients to a biosimilar in February and expects to save $300 million this year alone. Metal roof benefits drawbacks11/10/2023 Standing-seam metal roofs have wide, flat, vertical panels with a rib on each end and hidden fasteners underneath to create a sleek, modern appearance. Corrugated panels have exposed fasteners and a wavy pattern that makes installation easier. Metal panels are much larger and work best on more straightforward roof designs. Metal shingles are the smallest and are useful if you have a roof that requires fitting the material around chimneys and unusual shapes. Metal roofs come in a few styles, including metal shingles, metal panels, corrugated panels, and standing seams. The general rule is that a pitch steeper than 6:12 is steep enough to warrant extra labor. Roof pitches are measured in ratios of X:12, or the number of inches a roof rises vertically for every 12 inches it extends horizontally. A roof that is more slanted requires more materials, more installation time, and more safety measures for contractors. Your roof’s slope and pitch both affect material and labor costs. Even just a few hundred square feet can be a difference of a couple thousand dollars. The bigger your house, the more expensive a metal roof will be. Most roofing contractors price your installation based on your roof’s square footage. See our “ types of metal roofs ” section for a breakdown of each metal roof material and its average cost. For example, an aluminum roof is far more cost-effective than a copper or steel roof. There are many metal types to choose from, some of which are more expensive than others. Below, we’ll break down these factors and hidden costs like permitting and underlayment. The most significant cost factors for metal roofing are the roof’s material and size, but your home’s roof style and pitch will also determine the total cost. Get an idea of how much metal roof installation costs based on the table below.

Nova scotia duck tolling retriever oregon11/10/2023 Happily, like most hunter's dogs, the toller retains his training quite well, and does not require frequent re-training, as do most trainer's dogs. He must mix in plenty of marks and fun-dummies to keep the toller from becoming bored - and "creative" in how he repeats a drill. To succeed in drilling a toller, even within a positive framework, the trainer must be quite clever. In this they're like Chesapeakes, but then I think the two breeds share some common genes." "If you push too hard, they can become either cowed or stubborn. "Tollers respond best to positive training methods," Alison Strang said. However, the toller tends to resist the repetitive drilling required for the success in the advanced blind retrieves encountered in field trials and hunt tests. Happily, this same endurance comes in handy today when hunting to the gun in the uplands. This seemingly inexhaustible energy was necessary for tolling ducks all day back before bag limits. He has an almost insane desire to retrieve and will do it over and over and over without appearing to tire. He does most of what the generalist hunter needs quite naturally. To this day, the toller is more a hunter's dog than a trainer's dog. They developed the toller as a "hunter's dog," that is, a breed that does most of what the owner wants done naturally, with very little formal training beyond basic obedience. The toller was developed in rugged Nova Scotia by early Scottish immigrants, a hardy people who were scratching out a difficult living, struggling to survive in a strange new land. Not surprisingly, the golden is a "trainer's dog," that is, a breed that responds well to the repetitive drilling that extensive formal training requires. These aristocrats also had hired gamekeepers to manage their estates and train their dogs. The golden was developed on the British Isles by the landed gentry, who had huge estates on which to hunt. Different people developed the two breeds under different circumstances and for different purposes. And this reflects poorly on neither breed. He may look like a miniature golden, but inside, that is, under the hood, the two breeds are quite distinct. Never has the adage, you can't judge a book by its cover, been more true than in the case of the toller. In endurance, it excels most sporting breeds. In style, the breed rivals the English springer. These traits make him a great upland game finder/flusher/retriever. The toller is a bouncy, high-energy animal with amazing stamina. The color can be anything from light gold to coppery red. The double coat has a harsh water-repellent outercoat overlaying a dense wooly undercoat. Tollers stand about 17 inches to 21 inches and weigh 35 to 50 pounds, a nice size for today's hunting conditions. A white tip to the tail is highly prized by those who use these dogs for tolling, because when swished around it attracts the attention of rafted ducks. When you first see a toller, you might think it's a miniature golden retriever, except for the few flashy white points, perhaps on the chest, feet, and tail. See the first sidebar for background information on these knowledgeable folks. To help me present this multi-talented breed properly, I interviewed four long-time toller experts: Katie Dugger, Sue Dorscheid, Alison Strang, and Glen White. In fact, it's a masterpiece of misleadingly modest understatement. And wherever he has gone, he seems to be performing quite well, thank you.Ĭlearly, the name, Nova Scotia Duck Tolling Retriever, is not the breed's complete résumé. However, over the years he has become an international star, having been exported not only to USA, but also to various other countries around the world. Then, too, as the name indicates, the toller hails from Nova Scotia. See the second sidebar for information on tolling Oh, yes, I almost forgot: He can also toll ducks (and geese).

Saroo brierley book a long way home review11/10/2023 As a reader I want to experience joy, sadness, exhilaration and deflation but I felt none of these in this section of the book.Īgainst the odds, Saroo does find his home and he does have a very emotional reuniting with his mother. The writing is very factual and to the point. But…when he is trawling through the internet for hours on end, I’m just not so sure the reader is brought on that part of the journey with him. Saroo Brierley has an amazing story to tell. Now this is where I unfortunately lost interest in the book and found myself skimming over a few pages. He had a vague recollection of specific markers in his hometown in India and with the upgrading of Google Earth and technology over the years, the search became a little more accessible. With the development of the internet, and more importantly Google Earth, Saroo began a search that would remain his goal for many years. His adoptive parents did adopt another young boy from India but his was a different story to Saroo’s and his background, though sketchy, was a very traumatic one. But for Saroo it meant leaving his homeland, as the Brierley family lived in Australia.Ī warm welcome awaited Saroo and for many years he adapted well to this new culture, but always the longing to go home was there. Unable to locate his own immediate family and also due to Saroo’s young age and inability to communicate properly, it was felt that this new family would be his saving Grace. Saroo, through a pure twist of fate, ended up in the care of a very good lady, Mrs Sood, from the Indian Society for Sponsorship and Adoption (ISSA), who secured him an adoptive family. A little boy witnessing such scenes of a brutal nature when all he wants is to go home and get a warm and welcome hug from his mummy. Saroo continues with such vivid descriptions of the horrors that he was exposed to at such a young age and this was the part of the book that stole my heart. Daylight confirmed that this was the biggest river I had ever seen, but it was also fouler and smellier, lined with dead animals, human excrement and filth….I was horrified to see two dead people lying among the piles of rubbish, one with his throat cut, the other with his ears chopped off….The sight made me feel sick…it confirmed what I’d already begun to feel – that every day in this city was a matter of life and death.’ Saroo Age 5 ‘As the stallholders had seemed indifferent to begging children, I went along the water’s edge, thinking I might find people cooking there. Able to keep a low profile, Saroo eventually finds himself in Calcutta. In a blind panic he hops on a train, without any idea of where it travels to. The day passes and Saroo nods off but it is later when he wakes up, he discovers that he is alone. At the railway station, Guddu tells him to remain there while he heads off to look for food and work. One fateful day, Saroo persuades his older brother, Guddu, to take him along on one of his trips, as Saroo now feels old enough to handle the experience. At the tender age of five, Saroo looks after his two-year old younger sister while his mother and brothers go out to work and scavenge for food. The story of Saroo Brierley would break your heart. Unfortunately, for me, it did not live up to my expectations… In this case, I was very intrigued to read Lion, so I was only too delighted when it was chosen for our book-club read in May. The very interesting thing about being involved in a book-club is that you sometimes get to read books that you may not normally have gravitated toward. It celebrates the importance of never letting go of what drives the human spirit – hope. Then he set off on a journey to find his mother.Ī Long Way Home is a moving and inspirational true story of survival and triumph against incredible odds. And one day, after years of searching, he miraculously found what he was looking for. When he was a young man the advent of Google Earth led him to pore over satellite images of the country for landmarks he recognised. He spent hours staring at the map of India on his bedroom wall. Not knowing the name of his family or where he was from, he survived for weeks on the streets of Kolkata, before being taken into an orphanage and adopted by a couple in Australia.ĭespite being happy in his new family, Saroo always wondered about his origins. Saroo had become lost on a train in India at the age of five. When Saroo Brierley used Google Earth to find his long-lost home town half a world away, he made global headlines. Hardcoded hentai game11/9/2023 You could go the literary genius route, like in Corruption of Champions 2. Text-based games are all about the writing, and there are several ways to approach things. These kinds of games take finesse and talent. If someone else were to make this game, I’d recommend you don’t play it, probably. The interface tries to alleviate some of that stress, though. It’s not vital, and the entire game is rooted in the text. That’s why she added a graphical interface showing you some of the stuff that’s actually happening within the game. Aika knew that people are visual creatures, and they want to see some visual representation of what they’re fapping to. The reason this game and Trap Quest look like absolute dung is that Inform 7 doesn’t really welcome graphics, so the game is entirely text-based, in theory. This is actually a damn good game, even though the interface to it looks worse than an actual soiled diaper. It’s a game made for people who really like diapers, and I will never understand this fetish, dear God, why does it exist?Īnyways, I’ll get over my diaper hatred on my own time. You get stuck in diapers, and you have to shit in diapers, over and over again, while being raped or raping other people. Diaper Quest, though, has no transgender elements or sissification. That game is built in Inform 7, and so is Diaper Quest. In Trap Quest, you’re trapped in a VR world in which everything is trying to feminize you and turn you into a sissy or a transgender woman. But what does any of this have to do with Diaper Quest? Well… It’s basically a text-based simulator that lets you make text-based games. You can think of it as a subway route simulator that exists to let you build rooms that are interconnected. It’s built-in the Inform 7 engine, which is sort of a game engine, but not really. Trap Quest was and remains Aika’s greatest and most famous porn game. Now, this is a review of Diaper Quest, but it’s also technically kind of a review of her other famous game, Trap Quest.
